HONOREE & MEMORIAL DONATIONS
Make a donation in memory of a loved one, or to honor a special occassion: a good test grade, a lead role in a school play, a promotion at work, or help us raise money by asking friends, family and co-workers to make a donation in lieu of gifts for birthdays, graduations, weddings, anniversaries, etc.
If you mail a donation, please print and fill out the following form, leave the confirmation number blank, and mail the form with your check. If you make a donation via PayPal, please enter your PayPal confirmation number where indicated below and your honoree will receive an acknowledgment card notifying them that you made a donation in their name, along with a DZ Foundation wristband.
